Apply for Service Options Using Resources in a Community Environment (SOURCE)
This waiver program provides in-home and community-based services as an alternative to a nursing home.
- A Medicaid waiver provides recipients certain services not normally covered by Medicaid for a person with intellectual, developmental, or physical disabilities. These services can take place in the person’s home or in the community.
- Service Options Using Resources in a Community Environment (SOURCE) serves frail, elderly, and disabled Georgians. It provides coordinated services in their home or community.
- SOURCE can provide adult day care, alternative living services, personal care, home delivered meals, and respite care for family caregivers.

Make Sure You’re Eligible
SOURCE is only available in certain areas of the state. You must be a resident of a county served by this program.
To qualify, you must meet Supplemental Security Income (SSI) Medicaid eligibility criteria and:
- Be 65 or older or, if younger than 65, meet certain disability requirements
- Require a nursing home level of care

Submit Your Application
To apply for SOURCE, you must call the number listed for the provider serving your county. Once connected, a staff member will screen you by asking questions about your situation.
